- Click Send OTP button, OTP will be sent on the mobile, enter it and click “Click here to Verify OTP“
- Select “Birth” under Event Section of the form.
- Fill in the required details such as name, father name, mother name, date of birth, place of birth, state and other required details carefully in the form.
- Fill-in the captcha and click ‘Submit‘
- Upload the necessary documents such as proof of birth and identity proof.
- Pay the application fee online.
- Submit the application.
